Registered Nurse Learning Disability / Registered Mental Health Nurse

The post holder will be responsible for facilitating multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures. The post holder will adhere at all times to the relevant National Framework and guidance, NMC Code of Professional Conduct and work within established policies, procedures, protocols and systems ensuring quality standards and performance as directed by the Complex Case Manager /Clinical Commissioning Manager.
Main duties of the job
The post holder will be responsible for the day to day clinical and case management of a caseload of patients funded by NHS Continuing Healthcare (CHC) to include those jointly funded with the Local Authority. This will include assessment, review, and ongoing case management to ensure that care is safe and effective; in addition to ensure that it is funded through the correct funding stream when evaluated against the criteria set out in the CHC National Framework. You will work as an autonomous practitioner but also together with the team targeting specific elements of clinical review within the CHC service for Coventry, Rugby and Warwickshire.
Previous experience with CHC Framework would be desirable but is not essential. You will be fully supported through 1:1 supervision, a robust induction plan, and a friendly supportive team.

Job responsibilities
To facilitate multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures.
Support the Complex Case Manager/Clinical Commissioning Manager and the wider team to ensure that the workstream is planned and managed effectively and ensure their successful delivery.
To case manage your own clinical caseload (end to end) including, but not limited to; resolving issues with funding, placements, invoices and complaints.
To identify and support the arrangement of services required to meet the needs of individuals who qualify for NHS Continuing Healthcare and for the healthcare element of a joint care package as appropriate
To undertake investigations, as required, into an individuals eligibility for NHS Continuing Healthcare following an individual challenge or dispute.
To comply with the Data Protection Act 1998 and Code of Practice on confidentiality and data protection
To participate in ongoing professional development and clinical supervision in line with NMC guidelines
To ensure compliance with all statutory and mandatory training, Flexible, enthusiastic, reliable and self-motivated, with the ability to prioritise their workload
Be able to work under pressure and deliver to targets and deadlines.
They must also be able to demonstrate the ability to work through problems to a resolution
Have experience in communicating effectively with CHC colleagues, social services, care home providers, hospital discharge teams and other healthcare professionals and stakeholders.
Good IT skills are essential.
An understanding of working with care homes would be helpful, although not essential.

* Registered Nurse Assessor Role - Registered Nurse Learning Disability or Registered Mental Health Nurse
* Extensive Post Qualification Working Experience in Learning Disability or Mental Health Nursing
* Ability to Travel - Car Owner
* CHC Working Experience
* Experience of writing a Complex Clinical Report and meeting work deadlines.
