Main duties of the job
Your rolewill be to support the ongoing work within Cheltenham Central PCN, to delivermedicines optimisation and offer high quality, safe and cost-effectivemedicines and patient-centred care.
You will work closely with the PCN LeadPharmacists and Lead Technician to tailor the delivery of network prioritiesand targets with a practice-based focus.
You will work with the wider pharmacy technicianand pharmacist team to support in the provision of a consistent and reliable pharmacyservice to our PCN practices, 5 days per week. Main duties include medicinesreconciliation, supporting with the provision medicines information and adviceto patients and colleagues, audit and quality improvement work, monitoring medicinesto ensure safe prescribing and ensuring the practices are CQC compliant.
We are afuture focussed team and are keen to see proactivity as we develop the pharmacytechnician role. Our aim is to expand on the patient facing services that thepharmacy team and pharmacy technicians - can provide to support NHShealthcare and delivery of NHS (local and national) targets.

Job responsibilities
Job Summary
To work within the Cheltenham Central PCN,which includes 5 practices across Cheltenham, to support the Clinical Pharmacyteam in ensuring that medicines management processes within these practices areoptimised to achieve highly effective, safe and patient-centred prescribing.
The post holder will be a PharmacyTechnician, who acts within their professional boundaries, supporting andworking alongside a team of Pharmacists and other Pharmacy Technicians. In thisrole they will be supported by the Lead Clinical Pharmacists and Lead Technicianwho will develop, manage and mentor them.
The post holder will work as part of amulti-disciplinary team in a patient-facing role, with opportunities to workwith the Leads to develop the role of the Pharmacy Technician to further advancepatient facing roles. This is alongside the duties required of the pharmacyteam in terms of managing medications reconciliation and requests as detailed.
Alongside the Clinical Pharmacists, the postholder will provide support to general practice staff dealing with prescriptionand medication queries. They will help support the repeat prescription process,support with dealing with acute prescription requests, and carry out medicinesreconciliation on transfer of care. They will help implement systems for saferprescribing, providing expertise in clinical medicines advice while addressingboth public health and social care needs of patient in the GP practice withintheir competence.
Alongside the Clinical Pharmacists, the postholder will ensure that the practice integrates with community and hospitalpharmacy to help utilise skill mix, improve patient outcomes, ensure betteraccess to healthcare and help manage workload. The role is pivotal to improvingthe quality of care and operational efficiencies so requires motivation andpassion to deliver excellent service within general practice.
The post holder will ideally have undertakenthe CPPE Primary Care Education Pathway. However, if they do not hold these qualificationsthen training will be provided and supported.
Main Duties andResponsibilities
To undertake medicines reconciliation in accordance with practice protocols and toupdate patients medical records / prescribing accordingly. E.g. New patients,discharges and clinical letters.
To support the achievement of the practices prescribing Quality and OutcomesFramework targets (QOF), and Local and National Incentive schemes (e.g. PrimaryCare Offer).
Toprovide advice to GPs, staff and patients in changes to prescribing to supportthe improvement of prescribing safety, quality and cost effectiveness. AND
To take a proactive approach to the safe handling of prescriptions, includingassessment of risk and making recommendations for improvement
Forexample, MHRA alertmonitoring and action, High Risk Drug (including DOAC) monitoring and action(including teratogenic drugs actions needed and ad hoc as identified asclinically needed), CQC compliance searches covered, Liaising with Communitypharmacy and Hospital if errors identified, Liaising with community pharmacy resetting up dosette boxes for safe medication administration
To participate in medication audits.

Person Specification
Experience
* Experience of working as a qualified, registered pharmacy technician in primary care, community or hospital pharmacy
* Experience in working in primary care/ GP practice
Knowledge and skills
* Good clinical pharmacy knowledge including terminology
* Excellent communication skills, verbal and written, with the ability to adjust communication style and content to suit the audience
* Good time keeping
* Self-motivated and proactive
* Continued commitment to improve skills and ability in new areas of work

* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for the improvement of prescribing

Qualifications
* Professional registration with GPhC
