Outcomes Coordinator
An opportunity has become available to work within the Care4CE Dementia Reablement North Team for a Temporary Outcome Coordination for 32 hours a week. The team support referrals and their carers for people who have been recently diagnosed with dementia, the team offers advice and information to support the person to remain independent as possible, the team can offer support for up to 12 weeks using the reablement model. This is a temporary position for six months, working Monday to Friday for 6.4 hours a day. The service is office base is Macclesfield Town Hall, but referrals are from across the North part of Cheshire East. The Outcome Coordinator supervises a team of six Senior Support Workers to ensure the outcomes are met for the referrals.

About you
1. Experience of working a Social Care or Health Care setting.
2. Experience of supporting service users with dementia and carers who are new to social care services and developing good working relationships.
