About the Role
We are looking for an experienced and motivated Practice Nurse to provide high-quality patient care across a broad range of primary care services. This role focuses on long-term conditions (LTCs), acute care, immunisations, and health promotion. You will deliver evidence-based clinical care while contributing to the smooth running and quality of services within the practice and PCN.
Key Responsibilities
* Deliver clinical care in LTC clinics, vaccination programmes, acute care, wound care, contraception, cytology, and minor procedures.
* Follow NICE/NSF guidelines, participate in audits, and maintain infection control standards.
* Maintain accurate clinical records using SNOMED and manage your own CPD and revalidation.
* Collaborate with PCN colleagues and external agencies to provide seamless patient-centred care.
* Contribute to service improvement initiatives by applying clinical expertise.
Who We're Looking For
* Qualifications: Registered General Nurse (RGN) with current NMC registration; degree/diploma in nursing; specialist qualification in primary care or LTC care.
* Experience: Proven experience in primary care nursing, LTC management, immunisations, acute care, QOF/LCS delivery, audits, and infection control.
* Desirable: Independent/supplementary prescriber; postgraduate speciality qualifications.
