Specialist Nurse for Integrated Adolescent Service (IAS)

We are excited to offer an excellent opportunity for an SCPHN-qualified nurse to take on a newly created Specialist Nurse role within the Integrated Adolescent Service (IAS). This pivotal Band 7 role will lead the delivery of targeted health interventions for vulnerable adolescents involved in youth justice and safeguarding services across Lewisham.

Commissioned as part of an innovative two-year pilot, this post offers a unique opportunity to shape integrated, community-based care within a multi-agency environment.

Hosted within the Lewisham School Health Service and co-located within the IAS Health and Wellbeing Hub, the post holder will work as part of a multidisciplinary team, providing expert health input to support safeguarding, care planning, and service delivery. The role is central to improving access to timely, needs-led care for young people.

The successful candidate will undertake holistic assessments, identify unmet needs, and deliver targeted, trauma-informed interventions. They will play a key role in improving engagement with services, reducing health inequalities, and advocating for young people who are often hardest to reach.

The post holder will also contribute to service development, evaluation of the IAS pilot, and strengthening partnership working across health, social care, and youth justice.

Main duties of the job

* Undertake comprehensive, holistic health assessments for vulnerable adolescents, identifying physical, emotional, and developmental needs.
* Deliver brief, targeted health interventions (Tier 1) to support behaviour change and improve health outcomes.
* Provide accessible health advice and support, including sexual health, emotional wellbeing, substance misuse, and healthy lifestyles.
* Act as a health advocate, supporting young people to access appropriate services and ensuring continuity of care.
* Work collaboratively within a multidisciplinary team, contributing to safeguarding, youth justice, and care planning processes.
* Attend and contribute to multi-agency meetings, ensuring health perspectives inform decision-making.
* Maintain accurate clinical records and contribute to data collection, evaluation, and reporting for the IAS pilot.
* Deliver health promotion and engagement activities tailored to vulnerable adolescents.
* Provide specialist advice, consultation, and support to colleagues across health, education, and social care.
* Uphold professional standards, safeguarding responsibilities, and clinical governance requirements at all times.

Job responsibilities

This is a unique opportunity for a skilled SCPHN nurse to lead and deliver high-impact, community-based care for vulnerable adolescents within the Integrated Adolescent Service (IAS). The post holder will undertake holistic health assessments, identify unmet needs, and deliver targeted, trauma-informed interventions that improve health outcomes and reduce inequalities.

Working at a senior level within a multidisciplinary team, the role will provide expert health input into safeguarding and youth justice pathways, influence care planning, and strengthen multi-agency collaboration. The post holder will act as a key advocate for young people, improving access to services and driving engagement with those who are often hardest to reach.

The role also contributes strategically to service development, evaluation of the IAS pilot, and shaping future models of care, while maintaining high standards of clinical governance, safeguarding, and professional practice.

Person Specification


Qualifications and Training

* Degree or Postgraduate Diploma in Public Health Nursing (Specialist Community Public Health Nursing - School Nursing or Health Visiting).
* 1st level Registration with current NMC registration (Child, Mental Health, or Learning Disability).
* Evidence of post-registration training relevant to adolescent health, safeguarding, or public health nursing.
* Master's level qualification or equivalent experience in public health, safeguarding, or adolescent health.
* Training in sexual health, substance misuse, or neurodevelopmental assessment.
* Recognised teaching qualification (ENB 998/730/FLIPS or equivalent
* Evidence of further professional development


Experience

* Significant post-registration experience working with children and young people, particularly vulnerable or marginalised groups.
* Experience of assessing and managing diverse health needs, including mental health, substance misuse, and neuro-developmental conditions.
* Experience of multi-agency working, including safeguarding and risk management.
* Experience of delivering brief interventions and health promotion tailored to adolescents.
* Experience of working within Youth Justice, safeguarding, or custody settings.
* Experience of leading service development or contributing to evaluation frameworks.
* Experience of providing consultation, training, or supervision to colleagues in health, education, or social care.


Knowledge

* In-depth knowledge of adolescent health needs, including physical, emotional, and developmental factors.
* Understanding of safeguarding legislation, frameworks, and multi-agency processes.
* Knowledge of public health priorities and the Healthy Child Programme.
* Awareness of health inequalities and the needs of marginalised or underserved populations.
* Knowledge of trauma-informed practice and its application in adolescent health.
* Knowledge of local health and social care systems within Lewisham or similar urban settings.
* Familiarity with evaluation methods and outcome measurement in public health nursing.
* Understanding of sexual health, substance misuse, and neurodevelopmental pathways.
