Who You Are As a member of the HASS Cranbrook and Loxford team, you are part of a locality-based, multi-disciplinary team dedicated to delivering integrated community health and adult social care services. You are skilled in assessing, planning, and reviewing cases, handling approximately 4 cases per week with varying complexity. You bring strong experience working autonomously in multi-professional settings and are committed to person-centered care. What the Job Involves This role involves conducting complex occupational therapy assessments and delivering holistic, person-centered support planning and reviews for individuals with diverse health conditions and their carers. You'll manage a range of cases, making recommendations for adaptations and equipment, and ensure compliance with legal frameworks and housing legislation. The position offers an opportunity to engage in regular supervision and support, with considerations for flexible working arrangements. Skills Excellent knowledge of assessing and recommending adaptations and equipment Experience in multi-professional team environments Understanding of legal social care frameworks and housing legislation Ability to manage, plan, and prioritize complex cases Proficient in assessing and recommending both simple and complex equipment Professional occupational therapy qualification (HCPC recognized)
