CHC Nurse Assessor
Job Purpose:
The CHC Assessor is responsible for carrying out Continuing Healthcare assessments and reviews in line with the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care. The role ensures that individuals receive the right level of care and support, while contributing to efficiency initiatives and backlog reduction within the CHC service.
Location: Kent / Hybrid (2 days doing face 2 face in East-West Kent)

Duration of booking: Expected to last till April 2026 with possible extension
Proposed start date: ASAP
Pay Rates: Up to £270 a day
Hours / Working Days: 37.5 hours per week / Monday to Friday

Key Responsibilities:
Conduct Table-Top Assessments:
Review and assess CHC checklists, DSTs, and supporting documentation to determine eligibility through desk-based (table-top) exercises.
Face-to-Face Assessments:
Undertake in-person assessments with individuals, families, and multi-disciplinary teams to ensure accurate, person-centred decision-making.
Assist with Fast Track and Backlog Cases:
Support the CHC team in processing fast-track referrals and addressing CHC backlog cases in a timely and compliant manner.
Case Management:
Manage a caseload of CHC cases from referral to outcome, ensuring timely completion of documentation and effective communication with stakeholders.
Right-Sized Package Reviews:
Conduct right-sizing reviews of existing care packages to ensure they remain clinically appropriate, cost-effective, and aligned with individual needs.
Savings and Efficiency Exercises:
Contribute to CHC savings initiatives by identifying and implementing opportunities for improved efficiency and resource management.
