The Enhanced Support and Liaison Service (ESLS) is a newly formed service that supports adults with learning disabilities and/or autistic adults, who are:
* at risk of placement breakdown,
* at risk of hospital admission
* and/or need support to access mainstream mental health services.
We are looking for an Occupational Therapist with clinical and professional experience in working with individuals with Learning Disability and/or Autism. You will work alongside our lead OT to ensure that Individuals with Learning Disability and/or Autism with sensory needs and/or difficulties with activities of daily living and their support providers, benefit from a well co‑ordinated pro‑active, specialist, efficient and effective occupational therapy service.

Main duties of the job
You will provide professional advice and support across the three pathways within ESLS. The three pathways you will work are:
Enhanced and intensive community Intervention & Support
ESLS work with teams and services across the trust to provide support and intervention for those who are currently resident within Mental Health inpatient services, who are at risk of admission to these services or who are at risk of placement breakdown. This support is primarily through liaison, advice, training and consultation with other trust services and with wider system partners, including the Integrated Care Board (ICB), Lincolnshire County Council, United Lincolnshire Hospitals Trust, Police and Voluntary and Third Sector Organisations.
The service includes providing individual assessment, therapy and management, training, advice and a consultative approach. The focus of all support and intervention is to adhere to the ‘Building the Right Support’ Principles for this cohort of individuals within the NHS Long Term Plan – maintaining and supporting individuals to live well in the community, by reducing inappropriate hospital admissions and working to prevent placement breakdowns.

Person specification
Qualifications
* Qualified Occupational Therapist
* Evidence of specialised continued professional development in clinical practice, included accredited training
* Training in Sensory Integration
* LAPPLE accreditation
* Membership of relevant special interest groups
Experience
* Relevant experience of working with adults with complex needs or adults with mental health needs and skills that is transferable
* Significant experience of working with adults with Learning Disabilities (LD) and/or Autism, in community and inpatient settings
* Experience of working with people presenting with a wide range of complex needs including forensic presentation
* Experience of managing clinical risk and making autonomous decisions based on analysis of complex presenting problems
* In depth knowledge of reasonable adjustments
* Relevant experience of working with adults with Mild Learning Disabilities who have additional Mental Health needs
* Demonstrable evidence of substantial experience of working with individuals in crisis and skills that are transferable to both community / inpatient
* Good understanding of community resources
* Evidence of continuing professional development
Skills
* Sound knowledge of national agenda for Learning Disabilities, Autism and Transforming Care
* Highly developed clinical reasoning skills
* Highly developed skills in occupational therapy assessment and a range of interventions
* Sound knowledge of clinical/risk assessment and understanding of Information Governance principles
* Excellent written and verbal communication skills
* Excellent understanding of the therapeutic relationship and boundaries
* Delegation whilst maintaining overall responsibility for patients care, where appropriate
* Demonstrate the ability to lead a clinical team
* Highly motivated & able to engage with patients & carers to improve outcomes

* Additional knowledge and/or training in sensory processing

* Ability to travel around the county sometimes at short notice
