The Hillingdon Young People’s Transitional Service is offered to young people, with health needs and/or complex health needs who are preparing to leave children services. The Transition Nurse Specialist for children’s community team role is to facilitate, support and co-ordinate the transition from Children’s to transition to Adult Health Services. The role includes development of the transition service, participation in strategic expansion and collaborative working across health agencies, including Hillingdon Hospital, social care and education. The role includes a large educational component educating both Children’s and Adult Services to facilitate the transition process.
Main duties of the job
To take the lead in coordinating the transition process for young people with health and or complex health needs or life limiting conditions.
To compliment the current service provision within the Children’s Community Nursing Team Service and to work closely with the other members of the Transition.
To assess referrals to the Transition Service and where necessary direct referrer to a more appropriate service.
To ensure that a Health Summary/Health Passport is produced, in agreement with the young person, with identified health actions/health plan to be completed within a set time scale.
To assess the physical, social and psychological health and well-being needs of identified young people giving information, advice or counselling and support and making referrals as appropriate.
To provide specific advice, training and support to staff in health and other partner agencies who are providing care to young people in Hillingdon

Detailed job description and main responsibilities
You will have the ability to provide case management of an identified caseload of children and young people with complex health needs by utilising and promoting evidence-based care.
You should have excellent interpersonal skills to support you in managing complex discussions with a range of agencies and individuals, including care providers, patients and families.
In addition the ability to work in partnership with parents and multi-disciplinary agencies to provide skilled care to children and young people within the community to support safe transitioning to adult health services.
You will be highly organised and have a flexible approach to work, able to deal with difficult situations autonomously with demonstrable, excellent communication skills.

Qualifications
* NMC Registered Children's Nurse
* Diploma/degree or equivalent experience relevant to clinical practice
* Evidence of Continuing Professional Development
* Safeguarding training at level 3
* Dual registration in Adult & Childrens nursing or experience of working in both children's and adults service
Experience
* Substantial post qualification experience in a related area
* Knowledge and experience of collaborative multi agency work
* Knowledge and experience of teaching and assessing young people, carers and staff
* Knowledge and experience in community nursing
* Excellent computer skills incl. email, internet and office applications
* Knowledge and skills in the management of children and young people's health needs
* Sound knowledge of safeguarding children and vulnerable adults
* Experience of care planning
* Experience of changing practice
* Experience of working as a team member of a team but also able to work autonomously
* Experience of integrated or joint working to deliver a high quality or care and to achieve health outcomes
* Substantial post registration experience within a service working with children and young people with complex health needs
