Job Description
Overview:
Gateshead Council's Learning Disabilities Team is looking for a dedicated and experienced Social Worker to support adults with learning disabilities. The role focuses on person-centred assessments, care planning, and safeguarding to help individuals live as independently and safely as possible within the community.
Key Responsibilities:
* Carry out Care Act 2014 assessments and reviews for individuals with learning disabilities.
* Develop outcome-focused, person-centred support plans in collaboration with service users and their carers.
* Work within a multi-disciplinary framework, liaising with health professionals, care providers, families, and other stakeholders.
* Respond to safeguarding concerns and manage risks appropriately, in accordance with local policies and statutory guidance.
* Undertake Mental Capacity Act assessments and contribute to Best Interest decisions where necessary.
* Support transitions, hospital discharges, and reviews of long-term placements.
* Maintain accurate and timely case records using Gateshead's electronic case management system.
* Participate in team meetings, reflective supervision, and service development activities.
Person Specification:
* Qualified Social Worker with Social Work England (SWE) registration.
* Previous experience working in a community-based learning disabilities team or adult social care setting.
* Knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults procedures.
* Strong assessment, communication, and interpersonal skills.
* Ability to manage a varied caseload with competing priorities.
* Experience using electronic case recording systems is desirable.
* Enhanced DBS clearance is required.
