The successful candidate will be responsible for workingthrough their own rota and will undertake a variety of duties including surgeryconsultations, telephone consultations, queries, paperwork, signing repeatprescriptions and visiting people at home. They will be responsible forfollowing up their own investigations and will benefit from regular supervisionand reflection sessions with GP colleagues. The successful candidate will benefit fromworking alongside an established multi-disciplinary team including clinical pharmacists,paediatric nurse specialists, first contact physiotherapist and personalisedcare roles.


Main duties of the job

The GP will undertake avariety of duties governed by the GMS contract and objectives of EnhancedServices including:Share ofRoutine and Urgent appointments both face to face and telephonePrescribeas per local & national guidelinesShare ofhome visitsTime-managecompletion of administration: letters, results, tasks, referrals, telephonecalls, emailsTakeresponsibility for appropriate referrals (following local guidance and goodmedical practice) and administration pertaining to themParticipatein Extended Hours as agreed. Work withother health and social care providers to ensure a safe and integrated service.Provide comprehensive assessment, advice, and treatment for the registeredpractice population.Maintainaccurate and comprehensive records of consultations using SystmOne clinicalsystem.To provideclinical support to other members of staff, both clinical and administrative,in achieving the highest possible standards of care and QOF and EnhancedServices targetsThe contentof this post will be reviewed in consultation with the post-holder whennecessary and in line with the service developments.

Job responsibilities

CLINICAL

Recordingclear and contemporaneous I.T. based consultation notes to agreed standards.Providesafe, evidence-based, cost-effective, individualised patient care within thehub, surgery, patients own home.Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly referring patients to otherservices/agencies in a timely manner using local guidelines.Torecognise the need for and be able to perform and interpret investigatoryprocedures, including laboratory and radiology.Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare.Ensureappropriate follow up of patientsSupportpatients to adopt health promotion strategies that promote healthy lifestylesand apply principles of self-careSupport andwork with colleagues towards the delivery of anticipatory care plansRecogniseand work within your own competence and in accordance with professional codesof conduct, maintaining accurate and contemporaneous health recordsIndependentlyprescribe for patients within your scope of practice. Prescribing in accordancewith locally agreed or national guidelines.Reviewmedications following the appropriate policies, NICE/local clinical guidelinesand local care pathways.To maintainan awareness of developments in clinical practiceAwarenessof and compliance with all relevant practice guidelines e.g. prescribing, confidentiality,data protection, health and safety and QOF standards.Be aware ofyour duties and responsibilities regarding current legislation and adhere toour policies and procedures on Safeguarding Children and Safeguarding Adults.CONFIDENTIALITYYou willhave access to confidential information relating to patients and their Carers,practice staff and other healthcare workers. Experience

* Proven ability to evaluate the safety and effectiveness of own clinical practice.

* Understanding of policy developments related to the delivery of primary care services including General Practice, the GMS contract, Clinical Governance, Network Contract DES and Quality & Outcomes Framework

Qualifications

* Newly qualified GP (within 2 years and not having had a substantive post) with full GMC registration and license to practice.
* Certificate of Completion of Training in General Practice/Certificate Confirming Eligibility for General Practice Registration, or equivalent.
* Included on Medical Performers List
* Evidence of ongoing professional development
* Experience of working in a GP training practice using SystmOne.
