Acute Oncology Trainee ACP

The Advanced Clinical Practitioner (ACP) is a cutting-edgerole in the field of advanced clinical practice which crosses professionalboundaries, undertaking traditional medical roles and leadinginter-professional practice. It is a role model of excellence in the deliveryof holistic care to patients. On qualification, the ACP will be able to assessand treat many varied clinical cases; both acute and chronic. The post holderwill demonstrate critical thinking in the safe clinical decision-making processand is required to deliver an advanced level of clinical practice within thefield of speciality care.

Working within local and nationally agreed protocols andguidelines, the trainee ACP (tACP) demonstrates the knowledge and skills acrossthe Four Pillars of Advanced Practice. With support and supervision, the tACPexercises independent judgement to assess, investigate, diagnose, plan,implement and evaluate the clinical care and management of patients.

During their training, the post holder will develop anddemonstrate advanced skills and competencies and ensure that evidence-basedpractice is inherent in all aspects of care and treatment progressing to 8aupon completion.


Main duties of the job

The keyfunction of the Acute Oncology ACP is to assist with the delivery of acomprehensive acute oncology service that will link together Oncology GeneralMedicine and the wider Systemic Anti-Cancer Treatment (SACT) and haematologyservices.

You willwork as part of the Acute Oncology Team whilst completing your training,providing clinical advice for patients receiving oncology treatments e.g.Chemotherapy, Radiotherapy, Monoclonal antibodies, Immunotherapies, etc.

You willbe responsible for patients with newly diagnosed primaries of unknown originalong with those with newly diagnosed metastases requiring Oncologyintervention (in liaison with tumour specific CNS).

You willcare for and support patients who are experiencing an oncological emergency andensure they have the correct investigations and procedures.

You willact as the Oncology Specialist Nurse for patients with primary cancers,providing holistic assessment and co-ordinating the patients journey throughthe diagnostic and treatment phase of the patients journey, referring whereappropriate the key worker role to Primary care and / or Palliative care.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities

The trainee ACP will:

Deliverand participate in the clinical care of oncology patients attending the Trustunder supervision from a qualified ACP/senior clinician or designated lead.

Complete a3 year ACP MSc training programme at a local university education providerwithin the expected timeframe.

Helpdevelop the AOS service in line with regional and national targets andexpectations.

Prioritisehealth needs and intervene appropriately.

Beprofessionally and legally accountable for all work undertaken.

Practiceat an advanced level of professional autonomy and accountability that is withinthe Trusts policies and that of their professional regulatory body.

Takeresponsibility for the management of patients within own caseload andsupervision/development of junior members of the multi-professional team.

Be able toidentify the need for appropriate diagnostic tests and interpret the results.

Assist/leadin the on-going assessment, investigations, procedures, treatment andmanagement of patients in their speciality area.

Workwithin the multidisciplinary team to develop a portfolio of evidence ofdelivering advanced clinical skills and medical practice, clinical leadershipand providing clinical supervision/support and structured teaching andmentorship for multidisciplinary professionals (including medical staff) tofacilitate on-going education, training and development.

Toidentify and intervene proactively where circumstances contribute to an unsafeenvironment for patients, staff or relatives and escalate these to thedepartmental risk lead and/or consultant leads for clinical governance.

To shareknowledge and expertise, acting as an expert resource to others across primaryand secondary care, as appropriate.

Contributeto the development and improvement of systems and processes that facilitatepatient flow.

To improvethe quality of the patient experience by identifying and meeting the individualclinical needs of patients, including issues pertaining to child protection andvulnerable adults.

Once youhave successfully completed your Non-Medical Prescriber training, you will beable to prescribe / de-prescribe medicines for patients as per the legalrequirements for their professional regulatory body, National PrescribingCentre and national guidelines and in accordance with the Trusts non-medicalprescribing policy and Trust formulary.

Developthe skills to adapt specialist clinical knowledge and skills to differentclinical settings and influence service delivery and patient care.

To providespecialist advice on patient care to the healthcare team, taking into accountall aspects of clinical governance and act as a specialist resource within theTrust.

Screeningof patients for the early signs of disease and risk factors where appropriatefor role and department.

Responsiblefor rapidly evaluating the patients condition and response to therapeuticinterventions and treatment modalities, and modifies the plan of care foroptimal patient centred outcomes

Responsiblefor both initiating and then terminating an episode of care by either referringon to another agency or by discharging the patient under supervision ofconsultant/senior colleague.

Contributeto the review and development of clinical patient pathways to ensure astandardised approach to care.

Activelyimplement effective systems and processes for infection prevention and controlmanagement and relevant audit within their clinical area.

Educatespatients, relatives and staff as necessary.

Participatesin meeting and promoting the health education needs of patients and carers.

Appropriatelyassess, examine, investigate, diagnose and treat patients, resulting in thesafe and appropriate management of patients with undifferentiated,differentiated and complex presentations.

* UKONS Level 2 Acute Oncology Passport or working towards
* Advanced communication skills course
* Level 2 Psychology training
* Clinical skills course
* Non-medical prescriber
* UKONS Level 3 Acute Oncology Passport or working towards.
