**Senior Practitioner Role Summary**
This Senior Practitioner position plays a crucial role in ensuring the highest standards of service delivery. The successful candidate will be responsible for coordinating care transitions, mentoring staff, and upholding the principles of compassionate care.
**Key Responsibilities**
* Coordinate care transitions to maximize independence
* Provide direct care provision and staff mentorship
* Maintain optimal staffing levels and adapt work plans to evolving needs
* Connect service users to additional resources and services, empowering individuals to achieve their full potential
**Essential Requirements**
* SVQ Level 3 in Health and Social Care (or equivalent) or PDA in Supervision is desired
* Educated to N5 level (or equivalent by experience)
* Full UK driving license and access to a vehicle is required
* Registration with the Scottish Social Services Council (SSSC) is necessary within 3 months of commencement
