Role Summary
You will provide high‑quality, customer‑oriented support, working under the direction of the scheme manager and support team to deliver care, maintain dignity, improve health and wellbeing in a safe environment.

Role Profile – Core Responsibilities
* Assist in planning and delivery of personalised support and move‑on plans.
* Understand and apply the risk‑management process.
* Inspire and motivate customers to achieve agreed outcomes and develop life skills.
* Support day‑to‑day tenancy matters and improve tenancy readiness.
* Identify and promote opportunities for employment, education, and training.
* Signpost appropriate external support services.
* Support financial independence through budgeting plans.
* Coordinate healthcare needs and maintain contact with professionals.
* Empower medication self‑management following Riverside procedure.
* Maintain accurate, strengths‑based records on the digital platform.
* Encourage customer involvement and consultation.
* Deliver group work sessions.
Deliver a Support Service – Additional Duties
* Support the referral process for new customers.
* Contribute to housing management service, including income collection.
* Ensure customer safety and maintain a safe environment.
* Clean and prepare customer rooms as appropriate.
* Resolve difficult situations sensitively and safely.
* Act as point of contact over the phone and manage enquiries.
* Maintain local partnerships and ensure holistic support.
* Handle day‑to‑day administration and financial systems.
* Submit information returns on funding, health & safety, and performance.
Other Information
* Flexible working hours required, potentially outside normal office times.
* Use the Lone Worker system as necessary.

* Experience with vulnerable and diverse customer groups.
