An exciting opportunity has arisen for a Practice Pharmacist to join our dynamic and forward-thinking team at Brookside Group Practice. We are looking for 15-20 hours per week.
The Practice Pharmacist will be responsible for support to the practice with medication reviews, queries and the delivery of the repeat prescription service. Also, part of the group that implements effective medicine management within the practice, identifying areas for improvement, and initiating and managing change.
Main duties of the job
* Provide medication review and query services to patients via telephone or face-to-face clinics as appropriate.
* Ensure data is recorded (e.g., coding, reminders) so that patient care can be enhanced and data can be retrieved in a meaningful way to help manage patient care and support data analysis.
* Attend and contribute to various practice meetings as requested.

* Use of Anima to aid triage/consultation.
* Strong IT and systems including Emis Web, Accu-Rx, DXS, ANIMA Documents.
* Longstanding training practice with GP registrars, physician assistants trainees and others.
The Practice serves approximately 31,000 patients from our four surgeries in Earley, Lower Earley, Winnersh and Wilderness Road.
