Clinical Reconnect Practitioner
Location: Police Custody Suites or Courts - HMP Newhall
Department: Liaison and Diversion – Non-Custodial Team
The Role
As a Clinical Reconnect Practitioner, you will work autonomously across prison, community, and police custody settings to support individuals leaving prison within 12 weeks or up to 28 days post-release. Your goal is to reduce reoffending by addressing health and social needs and ensuring continuity of care through timely referrals and support plans.
Key Responsibilities:
* Conduct health screenings and assessments using validated tools
* Develop individualised support plans and make onward referrals
* Share information with partner agencies to ensure continuity of care and effective risk management
* Prepare accurate reports in line with quality standards
* Provide clinical supervision and support to junior team members
* Contribute Начис to service development, governance, and audits
About You
* Registered Mental Health Nurse, Mental Health Social Worker, or Occupational Therapist
* Current UK professional registration
* Experience working with patients with complex mental health needs
* Strong clinical assessment and risk management skills
* Knowledge of psychosocial interventions, substance misuse services, and multi-agency working
* Excellent communication, IT literacy, and problem‑solving skills
* Flexible, proactive, and committed to continuous professional development
