What you will do:
* Provide high-quality Occupational Therapy input for intermediate care beds.
* Collaborate effectively with a physiotherapist and care home staff to promote independence and rehabilitation.
* Facilitate safe and timely discharges home by effectively assessing and addressing patient needs.
* Demonstrate proficiency in manual handling and prescribe appropriate manual handling equipment.
* Work independently with remote support from the community therapy service.
* Travel to required locations as a car driver, ensuring timely and efficient service delivery.
Requirements:
* Must be HCPC registered.
* Car driver essential for the fulfilment of duties.
