The Role: To provide a safe and effective patient focused specialist clinical pharmacy service to a specified patient group(s) supporting medicines governance and quality improvement activities within specialist area(s) to ensure safe, effective and patient-centred systems in place.
Responsibilities
* Provide pharmaceutical advice for staff working within frailty
* As part of a rota provide clinical pharmacy cover over weekends and public holidays
* Undertake quality improvement initiatives and where appropriate liaising with key stakeholders
* Facilitate and support processes for medicines communication at the interfaces of care e.g. on discharge from a hospital ward or discharge to the care of the virtual ward or General Practice via an immediate discharge letter
* Liaise with relevant health and social care services as it applies to medicines (e.g. social work, acute hospital departments, General Practice, NHSL acute pharmacy services, Community Pharmacy and other key stakeholders)
* Delivery of relevant pharmacotherapy activities e.g. medicines reconciliation, education and quality improvement
* Participation in multidisciplinary consultant led ward rounds
There will be a focus on optimal use of medicines to ensure a patient focused, efficient, progressive, resilient, safe and effective service to frailty.
Qualifications and Requirements
* Masters of Pharmacy Degree
* Registered member of General Pharmaceutical Council
* Independent prescribing qualification or must work towards
* Evidence of postgraduate training and professional development in clinical pharmacy practice e.g. Post Registration Foundation Framework/ 1st year clinical diploma
* Relevant post registration experience in clinical pharmacy
* Ability to effectively monitor patients’ therapeutic regimens in order to ensure effectiveness and reduce toxicity
* Ability to work effectively as part of a team, in a pharmacy and multi-disciplinary environment
* Commitment to revalidation as per directions of General Pharmaceutical Council
* Good verbal and written communication skills
* Self-motivation
* Participation in weekends and public holiday service
* Participation in pharmacy emergency duty service
* Willingness to contribute to the general working of the pharmacy department
* Note: driving licence required (full UK/EU/EEA licence)
Desirable additional experience
* Demonstrable experience in providing a clinical pharmacy service to a defined patient group
* Experience in patient information/education
* Experience in supervising staff
* Clinical pharmacy/care planning knowledge and understanding
* Proven project, practice research and audit skills
* Ability to prioritise and organise workload

* Contract type: Permanent

* Hours: 37 per week
* Location: Pharmacy within University Hospital Wishaw
* Working pattern: 5/7
