We operate across three hospital sites in Warwickshire and Coventry, dedicated to responding quickly and efficiently and supporting older adults, their families and unpaid carers in a fast‑paced environment.
About the role
As a Social Worker you will join our established Hospital Team to provide a highly professional social work service. Your responsibilities will include managing a caseload, travelling throughout North and South Warwickshire as required, undertaking robust assessments and care & support planning, and communicating with customers, communities, health partners and agencies. You will work independently, often in pressurised situations, and receive regular supervision and day‑to‑day support from colleagues and team managers.
Key Requirements
* Professional Social Work qualification and registration with the Health and Care Professions Council.
* Availability to work a 5‑day week over 7 days, including one weekend per month in alignment with the 4‑weekly rota.
* Close collaboration with the multidisciplinary health & social care team.
* Full participation in team meetings and any additional meetings determined by the Team Manager or Service Manager.
* Driver’s licence essential for post‑discharge reviews, particularly in care homes.
* A dynamic, strengths‑based approach and the ability to make difficult decisions under tight deadlines.
* Experience in adult social care and good IT skills.
* Hospital social care experience desirable; willingness to learn in a fast‑paced environment.
* Familiarity with the Mosaic IT system (advantageous).
