HOURS: 30–37.5 hours/week
Salary: £14.50 per hour (negotiable depending on experience and qualifications).

Health and Wellbeing Coach / HCA
Purpose of the Role
Health and Wellbeing Coaches support individuals to take proactive steps in managing their physical and mental health, based on what matters to them. They help people build knowledge, skills, and confidence to improve their health outcomes and quality of life, supporting changes in health-related behaviours.
The role includes coaching individuals through multiple sessions, providing access to interventions such as self-management education, peer support, and guidance on using equipment such as blood glucose monitors. Coaches also perform Health Care Assistant (HCA) duties, supporting clinical teams with patient care, monitoring, and basic clinical tasks under the delegation of registered health professionals.
The successful candidate will be compassionate, reflective, and self-aware, with excellent communication skills. They should be proactive, flexible, and committed to personal and professional development.
Ongoing supervision, skills development, and support will be provided to ensure success in this role.
Key Responsibilities
1. Health Coaching
* Provide one-to-one and group health coaching sessions tailored to what is important to the individual, aiming to:
* Improve knowledge, skills, and confidence in managing health conditions.
* Empower individuals to set and achieve health goals.
* Support positive changes in health behaviours.
* Use a structured coaching framework to guide individuals through goal-setting, problem-solving, and skill-building.
* Support shared decision-making, self-management education, and peer support access.
2. Caseload Management
* Manage and prioritise a caseload according to patient needs.
* Identify when individuals require additional clinical or social support and refer appropriately.
3. HCA Duties
* Support clinical staff with routine patient care, observations, and monitoring as delegated.
* Assist with the use of medical equipment (e.g., blood glucose monitors, urine dip tests).
* Prepare patients for procedures, record observations, and update patient records.
* Maintain infection control and equipment standards.
4. Referral and Collaboration
* Promote health coaching and supported self-management across local healthcare services.
* Build strong working relationships with healthcare professionals and local agencies to ensure appropriate referrals.
* Encourage patient engagement with social prescribing and community support resources.
5. Gathering and Reporting Information
* Collect and record data on patient outcomes and service impact.
* Provide feedback to colleagues and referral organisations on patient progress.
* Work within data protection and information governance standards.
6. Supervision and Professional Development
* Participate in regular coaching supervision, mentoring, and ongoing learning opportunities.
* Adhere to organisational policies including confidentiality, safeguarding, equality, diversity, and health & safety.
7. Miscellaneous
* Work collaboratively with healthcare teams, other coaches, and support staff.
* Contribute to service improvement, risk identification, and business planning.
* Promote equality, diversity, and inclusion within the service.
* Undertake additional duties as required within the scope of the role.
Training
* Completion of Personalised Care Institute (PCI) accredited training for Health and Wellbeing Coaches is required.
* Ongoing training and development opportunities will be provided.
