Are you an experienced Social Worker - Mental Health Hospital Team.

If so, Randstad is recruiting for a Social Worker to join the Leicestershire County Council. This is a full-time role offering up to £34 per hour.
Here's the Role:
* Acting as the lead social care professional for a caseload of service users on acute mental health wards.
* Leading on complex discharge planning, ensuring safe, sustainable, and timely plans are in place.
* Taking lead responsibility for the assessment and coordination of Section 117 aftercare services, in close collaboration with health colleagues.
* Undertaking assessments under the Care Act 2014 and Mental Capacity Act 2005 to inform discharge plans.
* Working as an integral member of the multi-disciplinary team, contributing effectively to ward rounds, CPA reviews, and professionals' meetings.
* Liaising with community mental health teams, housing providers, families, and carers to co-produce robust aftercare plans.
* Providing advice and guidance on mental health legislation to clinical colleagues and service users.

Requirements:
* Social Work England registered
* Social Work Degree or equivalent
* Enhanced DBS
Experience:
* Adults Social Work
