Job Overview:
We are looking for an experienced and motivated Band 6 Physiotherapist to join the community team at University Hospitals Coventry and Warwickshire NHS Trust. In this role, you will deliver high:quality physiotherapy services to patients in the community, managing a caseload of individuals with a variety of musculoskeletal and rehabilitation needs. You will be responsible for conducting assessments, developing treatment plans, and supporting patients with their rehabilitation to improve mobility and independence.
Key Responsibilities:

Manage a community caseload, providing physiotherapy assessments and treatment plans for patients with musculoskeletal and rehabilitation needs.

Work collaboratively with multidisciplinary teams to ensure the best outcomes for patients.
*
Provide advice and education to patients and their families about self:management and rehabilitation strategies.
*
Develop and implement evidence:based interventions for a diverse range of conditions.
*
Maintain accurate and up:to:date patient records and documentation.
*
Contribute to the development of services and quality improvement initiatives within the team.
Key Requirements:
*
HCPC registration.
*
Minimum 6 months of post:graduate experience in community physiotherapy or relevant clinical settings.
*
Strong experience in musculoskeletal rehabilitation and complex case management.
*
Excellent communication and interpersonal skills.
*
Ability to work independently and as part of a team.
*
Driving license and access to a car for community visits.
