Responsibilities
* Provide evidence‑based care as a Chronic Condition Case Manager to individuals with highly complex needs, through disease‑specific training.
* Participate in a modular training programme to acquire the clinical skills required for a Nurse Practitioner role.
* Deliver clinically effective care within the District Nurse Team.
* Travel between patients’ homes in towns and rural areas, maintaining timely service delivery.
* Maintain patients in their own homes and communities, reducing hospital admissions and empowering self‑management.
Qualifications
* Registered nurse, Band 5 Community Nurse, with experience in district nursing.
* Excellent communication skills.
* Willingness to undertake further training and develop into a Chronic Condition Case Manager or Nurse Practitioner.
Travel Requirements
You must be able to travel between patients' homes within towns and rural areas in a timely manner.
Language Requirements
The ability to speak Welsh is desirable; Welsh and/or English speakers are equally welcome to apply.
