Job Opportunity: Health and Social Care Worker (HCSW) - Intermediate Care Team, St Andrews
An exciting opportunity has arisen for experienced HCSWs to join our Intermediate Care Team based in St Andrews.
About the Service
The Intermediate Care Team provides services to the adult population within the St Andrews area, delivered in service users' homes or as outpatient care when necessary.
Role Responsibilities
1. Work alongside Occupational Therapy and Physiotherapy staff to deliver therapeutic programmes in clients' homes, aiming to prevent hospital admissions and support early discharge.
2. Manage your own caseload, including assessment, progress, and discharge within set parameters.
Qualifications and Experience
Applicants should have an HNC/HND in Health and Social Care or equivalent, or SCQF level 8 qualification (e.g., Postural Stability Instructor). Experience as a therapy support worker and working with older adults is essential.
Skills and Attributes
Excellent organisational and communication skills, ability to work independently and as part of a multidisciplinary team, and flexibility to work across different community hospital sites are required.

* Public holiday working may be required.
* Regular supervision, PDP, and training opportunities are provided.
* Travel throughout the area is essential.
