Location: Lesley Clinic, Glenrothes

An opportunity has become available for a Frailty Practitioner to join our community complex care team. The role of the team is to support patients, carers and families in their home environment with a coordinated and case management approach to try and prevent unnecessary admission to hospital. Our patients have multiple long term conditions which contributes to complex health and social care needs. We are an integrated team and work closely with our health and social care colleagues. We lead the locality Multi-disciplinary Teams (MDTs) to support all our community colleagues.

You must be a influential leader and possess evidence of effective team working and have excellent interpersonal skills and be committed to person centered care. Good decision making, assessment skills and the ability to work with very minimal supervision are essential in this role as well as advanced verbal and written communications skills.

You will be expected to work autonomously utilising your own initiative, clinical judgment and decision making.

As this is a community post you will also be required to travel across Fife.

Please note the salary for this post is pro rata to part time hours.
