Overview
Community Integrated Care is seeking an experienced Health and Social Care Professional for the Service Leader - Level 2 position in Southport. The role involves leading a team of support workers, delivering person-centred care, and driving performance within services.
Responsibilities
* Lead and manage a team of support workers to deliver high-quality health and social care services.
* Drive performance, set pace and direction within services, and ensure services are appropriately resourced.
* Promote a positive and passionate culture and contribute to continuous improvement in care quality.
* Develop and maintain person-centred outcomes for people we support.
* Lead in safeguarding, risk management, and incident reporting as part of daily operations.
* Support training, development, and supervision of staff; contribute to recruitment and onboarding as needed.
Qualifications
* Hold a professional qualification in Health and Social Care (e.g., Nurse, Social Worker) or SVQ Level 3 plus supervisory units (Scotland) / Care Management Level 3 (England).
* Experience working in a health or social care setting with ability to support people to engage in their local and wider community.
* Experience working with individuals who require intensive support planning and management to ensure safety and quality of life.
* Ability to work with basic IT systems; training can be provided.
