Overview
General Medical Practitioner (Primary Care Network) role at NHS. We are looking for a GP to join our dynamic PCN multidisciplinary team to cover 6 sessions. The closing date is 19 October.
They will join our current PCN employed GPs to make a team of 3 PCN GPs. This growing GP team will contribute to our team of teams approach that connects the 7 practice teams with our PCN pharmacy, mental health, practice support, and social prescribing teams. The sixth session will be used to develop a PCN based portfolio career, pursuing interests that span population health and proactive personalised care. We have a number of PCN projects where a GP can slot in to contribute as part of our innovative approach to metabolic health, chronic pain, mental health or women's health.
Our PCN approach is to work closely in partnership with our local population to develop a collaborative community of support. This incorporates social prescribing, health coaching, care co-ordination alongside a wide primary care MDT (including mental health workers, pharmacists and pharmacy assistants, first contact physio, GP assistants and digital lead).
Main duties of the job

* Review of fixed term contract at 6 month mark (to review for permanent contract)
* Good supervision and mentorship with senior PCN GPs and managers

Job Description
Job responsibilities
In accordance with the agreed PCN timetable, the post-holder will undertake a variety of duties, including:
* Conducting surgery and telephone consultations, addressing patient queries, and visiting patients at home
* Checking, signing, and authorising prescriptions, while managing queries, paperwork, and correspondence in a timely manner
* Making professional, autonomous decisions regarding presenting problems, whether self-referred or referred by other health care workers
* Assessing the healthcare needs of patients with undifferentiated and undiagnosed issues
* Screening patients for disease risk factors and early signs of illness
* Developing care plans in consultation with patients, in line with current disease management protocols
* Providing counselling and health education
* Admitting or discharging patients from the caseload and referring them to other care providers as appropriate
* Recording clear and contemporaneous consultation notes to agreed standards
* Collecting data for audit purposes
* Attending MDT meetings
* Participation in education/teaching of wider MDT teams including medical and non medical teams
* Compiling and issuing computer-generated acute and repeat prescriptions, avoiding hand-written prescriptions whenever possible
* Prescribing in accordance with the Practice prescribing formulary or generically, as clinically appropriate
* Undertaking emergency patient visits during surgery hours, if clinically indicated
* Participating in on-call sessions and home visiting sessions as allocated in the practice rota
Qualifications and Experience
Essential
* Newly Qualified General Practitioner (with under 2 years from qualification) Registered General Practitioner (GMC)
* Current UK/EU Driving License
* Registered on local Performer’s List
* Willingness to complete an Enhanced DBS Check
* Up-to-date vaccinations
* Not previously been substantively employed at a GP Practice
* General understanding of the primary care contract
* Experience of working in a primary care environment
* Experience of continued professional development
* Experience of QOF and clinical audit
Desirable
* Experience of using Systmone/EMIS clinical systems
