Team Facilitator – Social Care Services
39 hours per week - 50% of your time providing direct support while the other 50% will be utilised for the management tasks of the team.
£28,961 per annum + additional On Call Payment

Hybrid role combining team management, direct support and practice leadership.
This role is to support Louise who lives in her own flat in Morningside. She is recovering from a difficult period in her life and is looking to get back to some of her favourite hobbies—going for long walks, dancing and visiting cafes and museums—along with lunches and coffees.
To support Louise to achieve this we have created a role for an experienced person to develop Louise's support. You will spend 50% of your time providing direct support and 50% on management tasks of the team. This will include practice leadership, auditing and implementing new strategies. Louise needs an active staff team that won't shy from a brisk walk, can be upbeat and able to cook. She requires structure, routine, and communication aids to navigate her day, and staff who can help her feel secure in stressful situations while promoting her independence and expanding her horizons.
Responsibilities
* Provide direct support to Louise for 50% of the working time, focusing on activities that promote independence and well-being.
* Lead and manage the frontline staff team, ensuring high-quality, person-centred support.
* Practice leadership, auditing, and implementing new strategies to improve outcomes.
* Create and deliver robust service designs, support strategies and risk assessments while managing individual budgets.
* Communicate effectively with Louise, staff teams, families and external care professionals.
* Maintain excellent organisational skills and prioritise workload under pressure.
* Ensure 50% of time is allocated to management tasks and team development.
Qualifications
* Experience working with adults and children with learning disabilities, epilepsy, autism and physical support needs.
* Experience in using person-centred planning techniques and delivering/leading excellent support practices.
* Strong facilitation skills to encourage staff ownership and responsibility for quality of support.
* Ability to communicate effectively with service users, staff, families and external professionals.
* Experience creating and delivering robust service designs, support strategies and risk assessments; ability to manage individual budgets.
* Excellent organisational skills and the ability to prioritise workload while working under pressure.
* SVQ Level 3 in Health and Social Care or equivalent.
* Full driving licence with access to own vehicle for business use.
