Job overview
The post holder will be an independent expert practitioner and will carry continuing responsibility for a defined caseload of HIV positive clients. This includes the setting of standards of care, the assessment of client needs, and the development, implementation and evaluation of a holistic program of health care which is evidence based. The post holder will be responsible for the delivery of advanced nursing skills within complex patient care pathways and will help lead the department in the development of clinical care systems, protocols and guidelines. This post will be responsible for providing and developing excellence in HIV clinical practice and be responsible for the teaching, mentoring and assessment of students and staff within the department. The post holder will ideally be a non-medical prescriber. They will work collaboratively with teams both within the health systems, and the community in order to provide high quality seamless service to clients /patients both in the hospital and community setting.
Main duties of the job
* Deliver expert clinical care to patients living with HIV across outpatient and community settings
* Undertake comprehensive patient assessments, including physical, psychological and social needs.
* Manage a defined caseload including complex cases.
* Lead and independently run nurse-led clinics, including monitoring, treatment support and routine follow up care.
* Provide counselling, education and adherence support.
* Work collaboratively with consultants, pharmacists and allied health professionals.
* Lead on health promotion initiatives, including prevention and partner notification strategies.
* Contribute to audit, research and continuous service improvement.
* Support junior staff and visiting students.

Detailed job description and main responsibilities
Work as an autonomous practitioner using higher level clinical knowledge and skills to manage individual episodes of care for both male and female patients, based on physical, social, psychological and emotional needs around their HIV diagnosis
Provide a nurse led clinic for stable clients, both on or off treatments Provide counselling and support and crisis intervention as necessary Provide support and education around treatment choice, informed consent, medication compliance and safer sex
Work closely with the Multidisciplinary Team, Pharmacists, Specialist Midwife and Consultants in order to provide a holistic service to HIV positive clients and their families both in hospital and the community
Work closely with the Sexual Health Service to ensure timely and appropriate partner notification and follow up
Demonstrate a high level of clinical decision-making and an ability to monitor and improve standards of care, working within his/her own scope of professional practice
Make and accept referrals to and from other disciplines, departments, other NHS trusts and voluntary agencies
Provide clinical leadership using higher level clinical skills, knowledge and expertise supervising others as appropriate
Use clinical evidence to inform and develop clinical practice within the HIV service
Support and develop health education and promotion strategies
Have highly developed communication skills to provide care that is culturally sensitive and meets the needs of a diverse population
Willing to undertake and utilise the independent nurse prescribing course
Assist in the maintenance of accurate client records using the client database
Act as an appropriate role model and expert resource to the MDT and other agencies as required
Support the Consultants and the Manager by providing professional leadership to the nursing and medical teams
Participate in developing, implementing and reviewing strategies for the development of clinical practice and education in HIV nursing /medicine.
Develop nursing practice in line with National, Trust and purchaser requirements
Develop nursing roles to advance the profession and HIV nursing practice
Work with colleagues to develop and communicate the vision of patient centered care
Work with the Management Team to improve care delivery through the development of clinical guidelines, policies and procedures including patient group directions and integrated care pathways.
Work with the senior nursing staff to review and support the development of the
HIV nursing service within the broader sexual health agenda
Facilitate, initiate, manage and evaluate changes in practice to improve quality of care
Act as a resource and provide appropriate support for all nurses
Support and promote the development and implementation of nursing and care services that reflect the principles of clinical governance
Support and initiate clinical risk management activities within their sphere of responsibility

* Significant post registration experience, including experience of working in a relevant clinical specialty
