Overview
This is a 3 month locum Consultant Respiratory post with 10 PA: 8x Direct Clinical Care, 1x SPA (revalidation) and 1x SPA to be agreed (sub-speciality lead/service development/leadership/governance). Inpatient and outpatient respiratory care; contribution towards respiratory out of hours on-call and weekend rota (1:10). Procedural competence including pleural, bronchoscopy and/or endobronchial ultrasound is advantageous depending on experience and sub-specialist expertise. Participation in clinical supervision for doctors in training and teaching medical students from Brighton and Sussex Medical School.
This is a full-time post, however part-time and job-share applications are welcome.
Main duties of the job
The clinical duties of the post include the following. Respiratory Direct Clinical Care (DCC) – 8 PAs. This includes a 1:10 respiratory on-call rota for weekends and an overnight on-call rota from home for specific respiratory advice. The acute medical take is run by the acute medical team daily seven days per week 08:00–20:00.
There are 3 consultants providing care for respiratory inpatients at any one time Monday to Friday, supported by the team of trainees. Daily consultant-led ward rounds are timetabled during ward weeks, covering inpatient areas: 2 consultants for Respiratory Ward C9 and 1 consultant providing a Respiratory Physician of the Week (RPW) model (Mon-Thu 08:30–17:00, Fri-Sun 08:00–16:00). RPW involves daily in-reaching to the acute floor to take over care of those admitted with respiratory disease, providing a referral service for inpatients under other teams, and providing rapid admission-avoidance reviews on the Respiratory SDEC or ambulatory care unit. The workload is shared with approximately 16–18 patients per consultant team.
There will be two outpatient clinics per week, including one general and one sub-specialist respiratory clinic, to be determined by the skills and expertise of the successful applicant.

Job responsibilities
The UHSussex (East) respiratory department provides specialist services for inpatients and outpatients at RSCH and Princess Royal Hospital. There is an in-patient consultation service during normal working hours Monday–Friday, and an on-call service for out-of-hours NIV management across both sites.
RSCH details include the Respiratory ward (C9) with 20 single en-suite rooms, 2x 4-bedded high acuity bays, a 6-bed escalation area, a Respiratory SDEC area and a procedure room. Ward duty is shared among 3 consultants and supported by trainees, clinical fellows and specialist nurses, with daily rounds at 08:30.
Out of Hours commitment is 1 in 10 weekends covering RPW, C9 ward and outliers. There is also a Respiratory Virtual Ward for telephone advice in evenings and weekends.
There are established sub-specialty services including ILD, non-CF bronchiectasis, TB, sleep, NIV/CPAP, lung cancer and pleural disease, with teaching and supervision roles for trainees. A range of MDTs and clinics are held to support patient care across primary and secondary care.
On calls and clinical duties are aligned with the integrated care/virtual ward model and the Respiratory SDEC/ambulatory care pathways. The post includes opportunities to participate in pleural and bronchoscopy/EBUS lists dependent on experience.
There is a commitment to clinical supervision and teaching for medical trainees, with opportunities for educational supervision and involvement in regional procedures such as endobronchial ultrasound.
Requirements
Essential and Desirable elements from the candidate specification include:
* Basic medical degree and full GMC Registration with licence to practice
* CCT in Respiratory Medicine and General Medicine; entry onto the GMC Specialist Register
* Appropriate postgraduate qualification and/or higher degree (Desirable)
* Experience in respiratory medicine, unselected medical take and care of general medical in-patients; understanding of clinical risk management
* Sub-specialty respiratory medicine experience/training (Desirable)
* Experience working in multi-professional teams; understanding NHS governance and resource constraints
* Teaching commitment with evidence of teaching activity; recognised teaching qualification (Desirable)
* Evidence of audit, potential for research activity; publications and awards (Desirable)
* Good communication skills (essential)
