Advanced Practitioner – Leaving Care Service The London Borough of Havering is looking to expand its current team with a dedicated and experienced Advanced Practitioner seeking an opportunity to join a well-structured and organised Leaving Care Service. This Advanced Practitioner role will initially be for 3–6 months, with potential for extension subject to internal circumstances and performance. Responsibilities include: Supporting and mentoring Social Workers and Personal Advisors within the Leaving Care Service. Holding a reduced caseload of complex young people and providing practice leadership across the team. Completing high-quality Needs Assessments, Pathway Plans, risk assessments, and statutory visits for young people leaving care. Ensuring all key performance indicators are met, including pathway plans, reviews, case recordings, chronologies, and health-related assessments for young people aged 16+.
