Overview
An exciting opportunity has arisen to join the District Nursing Service in Northumberland for a Community Staff Nurse. This is a fixed term 12 month contract for secondment cover within the team, based in the West Northumberland area within the Ponteland Team. The role involves providing patient-centred care to patients in their own homes, residential care settings and in clinics, as part of a district nursing team that values professional development and safe service delivery in line with Trust values.
Community staff nurses work autonomously and as part of a team, reporting to their line manager, the community nursing team lead, and are responsible for assessing, planning, implementing and evaluating care for a diverse group of patients with a wide range of conditions and health needs.
Responsibilities
* Development and delivery of programmes of health and social care.
* Delivery of care and support to patients with a terminal illness and their families, including emotional and bereavement support.
* Performance of skilled nursing care including assessment, wound management, palliative care and catheterisation.
* Performance of diagnostic tests such as Ankle Brachial Pressure Index using Doppler and interpretation of results to decide further action.
* Performance of procedures requiring fine dexterity and skill such as venepuncture, drug administration by injection and removal of sutures.
* Interpretation of results and communication to patients (e.g., blood glucose estimation); explain aspects of care to patients and carers and address questions and concerns.
* Maintenance and promotion of high standards of professional practice and sharing approaches with others.
* Maintaining a duty of care to patients, carers and colleagues through personal and professional accountability.
* Providing care in line with the Trust's local services, including care at home, in clinics and in hospital settings managed by the Trust.

We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics.
