Main duties of the job
* Work as an empathetic health coach providing personalised, holistic care and support to patients through a mixture of video consultations and chat messaging.
* Monitor and support a group of patients and, working under defined protocols, flag the need for expert clinical input as and when required.
* Provide active monitoring, support and motivation to patients, without providing advice.
* Implement, monitor and review patient programmes in line with standard operating procedures.
* Organise and prioritise your designated workload.
* Effectively communicate with colleagues, patients and their relatives/carers, liaising as required with referring clinicians and other members of the clinical team, both verbally and in writing.
* Coordinate and organise appointments with autonomous diary management.
* Ensure the information needs of patients are met, providing appropriate resources, and referring to specialists where relevant.
* Accurately record and capture coaching interactions.
* Document and monitor all aspects of care coordination and service delivery.
* Support and participate in data collection for research and audit.
* Engage in regular reflective practice and training.
Experience
* Professional experience in using behaviour change techniques to support improvement of health and wellbeing.
* Professional experience of working in a healthcare environment or similar experience.
* Experience coaching clients and / or carers.
* Comfortable using evolving technology and multiple different systems.
* Excellent communication and inter-personal skills.
* Passionate about helping people get healthy and fit.
* Ability to show empathy, compassion and understand difficulties faced by people affected by cancer.
* Ability to multi-task and manage competing priorities simultaneously.
* Strong team working and collaboration skills.
* A positive can‑do attitude and willingness to get stuck in, help out and deliver outstanding person‑centered care.
* You are flexible and adaptable.
* Able to work in an ambiguous and fast paced organisation and enjoy being part of a rapidly expanding company.
* Excellent written communication and ability to communicate sensitively to patients via chat messaging.
* Experience of working with people with cancer.
* Evidence of multi‑professional working.
Qualifications
* Degree educated in a relevant subject (for instance; relating to health or wellbeing).
* A coaching qualification.
* Motivational Interviewing skills.

Salary: £29,500‑£31,500 depending on experience.
