The Hospital Team is conveniently based at Churchill House in Solihull town centre, as part of this highly experienced and dedicated team, you will embed a strengths-based model of assessment and support, promoting independence and well-being and achieving better outcomes for people, carers and their families. You will ensure that there is a strong Adult Care and Support focus while working in partnership with health colleagues and care providers such as reablement. Within both teams, we also offer comprehensive training with opportunities to study to be a Best Interest Assessor or Practice Educator which provides a positive base for supervision skills. Knowledge of safeguarding principles and relevant legislation. Experience of delivering person-centred and strengths-based working including assessments and reviews. Detailed knowledge of the care legislation including the continuing health care framework. Experience of assessment, the development and planning of care and of positive risk management. Effective communication skills and experience of collaborative working to build effective working relationships. Above all, a passion for supporting adults with disabilities to live fulfilling lives and maximise their opportunities for independence.
