Care Coordinator

Are you a dynamic and enthusiastic Mental Health Nurse, Social Workers or Occupational Therapists with a good knowledge of psychosis and a passion for the Early intervention ethos? Then we are looking for YOU!

CAMEO, the award winning service, operates across the Cambridgeshire and Peterborough Foundation Trust via two established bases in their busy cities.

This post will be located in the dynamic and multi cultural city of Cambridge where we are seeking to recruit highly trained and hard working Band 6 Care Coordinators: Mental health Nurses, Social Workers or Occupational Therapists. The practitioner will provide specialist advice, assess, plan and implement care to a designated caseload whilst working at all times to promote the EI ethos. They will ensure the safety and well-being of service users and their families/carers through family intervention models.

The individual must be compassionate, committed and have good working knowledge of psychosis displaying a passion about the values thatunderpin EI work and supporting service users throughout their recovery journey's.

Main duties of the job

The post holder will be an autonomous practitioner and responsible for a defined caseload of service users.

The ideal candidate will have a strong background in mental health, be a team player with a great work ethic; a sense of accountability, accompanied by an ability to work under pressure and a desire for innovation.

We are able to support agile working for staff, using all appropriate I.T equipment and support.

To undertake assessments of individuals and their families in the designated care group with complex health presentations, includingthose service users presenting with higher levels of risk.

They will work alongside the service user to develop their care plan along the recovery principles.

To offer specialist assessment and advice where required.

To assess, manage and regularly review on going risks as identified during the assessment, ensuring that this is done in collaborationwith the service user and carers.

To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedures.

To provide supervision of junior staff and trainees where appropriate (mentorship and preceptorship qualifications).

To act within the relevant Code of Conduct dependent on your Registration Body.

Person Specification


Education and Qualifications

* Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
* Registered Social Worker OR, Registered OT
* Completion of Mentorship Course/ENB equivalent, or intention to complete.
* Evidence of continuous professional development


Experience

* Significant post qualifying experience in working with those presenting with serious mental illness
* Experience of working alongside people with complex health problems in the designated care group
* Experience of managing a caseload
* Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
* Experience of conducting clinical assessments including risk assessments
* Experience and knowledge of dual diagnosis/substance misuse
* Previous experience of working in the community


Skills & Abilities

* Excellent written and oral communication skills
* Ability to develop effective professional relationships with others
* Develop own skills and knowledge and provide information to others to help their development
* Skills of assessing and interpreting service user conditions with appropriate actions
* Plan and organise complex activities or programmes requiring formulation and review
* Able to use Word, PowerPoint, clinical systems, email and /or willingness to learn
* The ability to multitask and mental adaptation to different unpredictable situations
* oParticipate in Partnership working with individuals, groups, communities and agencies
* oPromote people's equality, diversity and human rights
* oSkills of assessing and interpreting service user conditions with appropriate actions
* oPlan and organise complex activities or programmes requiring formulation and review


Other

* Ability to travel across sites
* Punctual and flexible across hours of work when required.
