Overview

An exciting opportunity has arisen for dynamic and motivated registered nurses to join our highly skilled, innovative, and rapidly expanding Frailty service. The post holder will be mainly based on Bevan Court 1 but will also be expected to support the frailty service in other locations.

The Frailty Service comprises of:

* OPAL (Older Persons Assessment and Liaison Service): This twenty-four bedded unit is geriatrician led and open 24/7 to provide an overnight stay/short stay area for those patients living with frailty who need further review and treatment.
* A dedicated Ambulatory unit: provides rapid Comprehensive Geriatric Assessment (CGA) for patients with frailty and frailty syndrome.
* A 16 bed in-patient Frailty Assessment ward: benefits from daily Consultant Geriatrician leadership along with a dedicated team of nursing, therapy, and social care professionals.
* A Community Frailty Service for St Helens CCG: provides CGA, post-discharge follow up, and ongoing management for frail patients in the community.
* An Urgent Community Response team: provides a 2-hour response for urgent health, social or therapy input.
* Frailty Virtual Wards: provides a hospital-at-home service for patients living with frailty in the community.


Responsibilities

* To be responsible, in association with the team leader, for the formulation and implementation of individual patient care plans.
* To participate in the holistic assessment of patients, formulate and review individual care plans, and act as 'named nurse' or associate for a group of patients from admission to discharge as determined by the Ward Manager.
* To commence and participate in research/special projects as directed by the ward manager.
* To contribute to the setting, maintaining and monitoring of standards of care within the ward.
* Help to ensure that all equipment is in safe working order in conjunction with the department equipment controller.
* Internal rotation onto night duty and twilight shifts is required.
* To assist in the promotion of effective communication with all ward staff and other departments as required.
* To remain updated and fully comply with all Trust infection control policies.
* Promote the safeguarding of vulnerable patients in line with national and local policy.
* Provide and receive complex information, communicating sensitively and confidentially (e.g., care plan discussions with patients).
* Communicate in a compassionate and empathetic manner, using persuasion and reassurance.
* Identify barriers to communication and modify communication style as required.
* Effectively communicate while providing training to students.
* Demonstrate effective communication skills with the wider MDT team.


Clinical Skills

* Plan and organise a defined caseload of patients on a continuing basis.
* Assess, plan, implement/deliver and evaluate patient care for a defined caseload and recognise signs of deterioration/change and respond according to protocol.
* Maintain appropriate levels of training for clinical skills as needed within scope of practice.
* Maintain safe custody of drugs, ensuring that checking, witnessing and administration procedures are compatible with statutory and trust requirements.
* In the absence of the Sister/Charge Nurse, effectively respond to complaints/concerns and escalate as required.
* Ensure clinical incidents/near misses are managed and reported and escalated in a timely manner.
* Be part of the multidisciplinary team decision making, acting as the patient’s advocate in order to support the patient’s pathway.
* Work within organisational policy, SOPs and guidelines.
* Undertake risk assessments and implement risk reducing measures; report and escalate to the Line Manager any deficiencies in minimising risk.
* Ensure privacy, dignity and safety of individuals is maintained at all times.


Use of Resources

* Actively participate in the effective and efficient use of ward/departmental resources and equipment, including stock management and care and maintenance of equipment.
* Handle patients’ valuables, ensuring safe storage and transportation.
* Sign off agency timesheets to verify and record hours worked.
* Liaise with the Roster Coordinator to balance personal roster within the roster period.
* Order and receipt of goods in accordance with the Trust's financial framework.


Team Development

* Deputise in the absence of the Sister/Charge Nurse.
* Manage own time and that of others through delegations to ensure high quality service delivery.
* Act as assessor/supervisor for junior staff and students.
* Participate in setting yearly personal and professional objectives for self.
* Support the department's appraisal process for junior staff by providing feedback.
* Be flexible in the delivery of safe staffing across the Trust and participate in redeployment where within scope of practice.


Research and Audit

* Contribute to data collection for research and audit purposes.
* Identify audit topics relevant to the ward/department.
* Critically evaluate research before applying it to clinical practice to enhance patient care.


Personal Development

* Identify areas for self-development, interests, and training requirements within personal performance planning and development review.
* Maintain a professional portfolio and reflective diary for developmental purposes and revalidation.
* Support the rest of the team by recognizing and promoting self-help and wellbeing.
* Develop a culture of learning and innovation, creating high-quality learning environments.
* Adopt a reflective approach to practice with a view to continual improvement.
