Coordinator for Additional and Enhanced Access Services x2

Job Title: Care Coordinator

Salary: £31,792 per annum

Hours: 37.5 hours per week

Reporting To: Team Leader


Key Responsibilities

* Admin duties include SystmOne related tasks such as ICE, pathology reports, sorting incoming post to be attached to the relevant medical record and creating tasks for GP practices as advised by clinicians
* Assist patients with appointment check‑ins, new appointment/rescheduling and queries
* Assist clinicians with admin duties, communicating with them in person and via SystmOne messenger
* Call patients to confirm bookings/send SMS confirmations
* Complete clinical stock checks and order stock as required; complete clinic room and fridge checks
* Complete 2WW and other routine/urgent referrals to the relevant service
* Manage clinician bookings via My Locum Manager (MLM); monitor team mailbox for booking updates
* Maintain patient data privacy and support wider HCL organisation
* Reception desk duties including inbound/outbound telephone calls and responding to emails
* Contribute to policies and SOPs; update contact information and rota on SystmOne
* Proactively engage in the development of the Access Hubs and South Westminster Health Centre
* Protect patient data/privacy and provide administrative support to the wider HCL organisation


Requirements

* Computer literate, proficient with Microsoft Office and NHS mail; able to learn relevant software/tools.
* Excellent verbal and written communication skills; confident speaking to service users and clinicians face‑to‑face and via telephone.
* Good knowledge of primary care, local services and NHS digital programmes.
* Experience working in a GP practice or patient‑facing admin/receptionist role.
* Strong attention to detail, time management and independence in work.
* Experience with SystmOne or EMIS software.
* Degree level education in a relevant subject.
* Ability to work under supervision to agreed deadlines, adapt to change, and manage challenging conversations with emotional resilience.
