An exciting opportunity has arisen for an enthusiastic clinical pharmacist to join our forward‑thinking, patient‑focused Orthopaedic team to provide an enhanced service for orthopaedic elective and trauma patients.
The post holder will join a team responsible for the pre‑operative assessment clinics and prescribing/transcribing services on the orthopaedic inpatient wards.
Responsibilities
* Perform pre‑assessment of patients in orthopaedic elective and trauma settings
* Provide counselling to patients about medication and therapy
* Write electronic drug charts in preparation for patient admission
* Optimize medicines therapy for patients throughout admission
* Prescribe for all inpatients and complete electronic drug charts for emergency admissions
* Write discharge prescriptions and ensure timely provision of discharge medication
* Conduct therapeutic drug monitoring and adjust therapy accordingly
* Lead audit and quality improvement projects within the orthopaedic pharmacy service
* Coordinate with the SDAU pharmacy team to update medicines on admission to the ward
* Act as a specialist pharmacist providing an advanced level clinical pharmacy service to high‑risk patient populations within orthopaedics
* Initiate and implement service developments such as one‑stop dispensing, patient self‑administration schemes, and use of patients’ own drugs
* Develop specialist knowledge of the clinical speciality and treatment of co‑morbidities to promote evidence‑based medicine across the Trust
* Suitable applicants who are not prescribers will be considered
