We are looking for a Healthcare Support Worker to join our team of Occupational Therapists and Physiotherapists based within Dundee Community Rehabilitation team. We are part of a friendly, hard working and dynamic integrated team, with physiotherapists, Dundee City Council Occupational Therapists, the Integrated Living Officer and Store Manager. The Dundee East and West Rehabilitation teams work closely to deliver a service to the people of Dundee. We are located across two sites and you will have the opportunity to develop your knowledge and skills and be responsible for maximising the functional potential of clients. This team routinely receives referrals from hospitals within Dundee, GP practices, other health and social care providers, third sector providers and self-referrals from clients.
You will be expected to hold a diverse caseload, which includes clients with a range of diagnoses. We use an individualised rehabilitation approach to promote independence and quality of life, linking with statutory and third sector services to ensure a holistic approach. You will provide a supportive role to your Physiotherapy and Occupational Therapy colleagues working within Community Rehabilitation team.
Responsibilities
* Contribute to an integrated team approach within Community Rehabilitation, supporting Occupational Therapy and Physiotherapy colleagues.
* Manage a diverse caseload with a range of diagnoses, applying an individualised rehabilitation approach to promote independence and quality of life.
* Maximise the functional potential of clients and link with statutory and third sector services to deliver holistic care.
* Be involved in service delivery across two sites and contribute to developing knowledge and skills within the team.
* Participate in referrals from hospitals, GP practices, other health and social care providers, third sector providers and self-referrals.
* Understand the roles of Occupational Therapy and Physiotherapy and operate within a health or social care setting.
* Achieve the mandatory Induction standards for Healthcare Support Workers in Scotland and comply with the Code of Conduct throughout employment.
* Recognise the move towards 7 day working as part of a 5 out of 7 day rota to enable a person-centred equitable approach to care.
Qualifications
* Understanding of the role of Occupational Therapy and Physiotherapy and experience in a health or social care setting.
* Ability to work within a multidisciplinary team and in a changing rota pattern.
* Commitment to mandatory induction standards and professional conduct.
