Job Overview
We are looking for a registered nurse to join the Crawley Healthy and First Community Primary Care Network Team based at Crawley Hospital covering Crawley and Horley. The role focuses on diabetic management, wound management including leg ulcer diagnosis and treatment, palliative care, bladder and bowel care (urinary and supra pubic catheters and continence needs), and working in partnership with local health and social providers for people under frailty. The goal is to deliver high‑quality care in the community, supporting patients towards rehabilitation, self‑management and independence throughout the continuum of care.
Responsibilities
* Provide diabetic management and support patients with blood glucose control.
* Diagnose and treat wound management, including leg ulcers.
* Deliver palliative care and end‑of‑life support.
* Manage bladder and bowel care, including catheter care and continence needs.
* Collaborate with local health and social providers to support people under the frailty pathway.
* Facilitate evidence‑based care and support staff learning and development.
* Assist patients in rehabilitation and self‑management to achieve independence.
