Overview
The Care Coordinator plays a vital role in ensuring that patients receive the appropriate care and support throughout their healthcare journey. This position involves liaising between patients, healthcare providers, and other stakeholders to facilitate effective communication and coordination of services. The ideal candidate will possess strong organisational skills and a keen attention to detail, ensuring that all aspects of patient care are managed efficiently.
Responsibilities
- Serve as the primary point of contact for patients, addressing inquiries and providing guidance on care options.
- Coordinate appointments, referrals, and follow-up care to ensure continuity of services.
- Maintain accurate and up-to-date patient records, documenting interactions and care plans.
- Collaborate with healthcare professionals to develop individualised care plans tailored to patient needs.
- Monitor patient progress and adjust care plans as necessary, ensuring optimal outcomes.
- Assist in the management of office operations, including scheduling and administrative tasks.
- Provide support to patients and their families, offering resources and information about available services.
- Participate in team meetings to discuss patient cases and share insights for improved care delivery.

Experience
- Previous experience in a healthcare setting is preferred, with a strong understanding of patient care processes.
- Proficiency in office management tasks, including scheduling, record keeping, and communication with various stakeholders.
- Excellent interpersonal skills with the ability to build rapport with patients and colleagues alike.
- Strong organisational abilities, capable of managing multiple tasks simultaneously while maintaining attention to detail.
- Familiarity with electronic health records (EHR) systems is advantageous but not essential.
