Location: Maidstone Medical Practice, part of the Kent Combined Medical Practice (Maidstone, Chatham and Shorncliffe).

Main duties of the job
The post holder will work as part of a multidisciplinary primary healthcare team delivering nursing care across all three sites of the Kent Combined Medical Practice. Responsibilities include assessing and diagnosing patients with complex health needs, minor injuries and illnesses, managing chronic conditions, and coordinating clinical services.
Job responsibilities
* Providing safe, evidence‑based, cost‑effective and individualised patient care.
* Managing the care of patients with chronic illnesses (e.g. asthma, diabetes, chronic heart disease, hypertension) and maintaining the Chronic Disease Register.
* Undertaking health and well‑being clinics such as smoking cessation, weight management and alcohol misuse clinics.
* Having direct responsibility for patient caseload, ensuring accurate plans of care and incorporating recommendations.
* Coordinating diagnostic tests, treatment plans, health promotion and prevention strategies.
* Supporting the development of specialist services and acting as a regional resource for advice and regional clinics.
Qualifications
* Postgraduate degree/diploma in primary healthcare or active pursuit of equivalent qualification.
* Learning and Assessing in Practice Qualification or equivalent practice assessment training.
* Valid registration with the UK Nursing & Midwifery Council (NMC) and current membership number.
* Basic Life Support (BLS), Anaphylaxis, Automated External Defibrillation (AED), Infection Prevention & Control (IPC) and Safeguarding Level 3 (children/adult as required).
* Registered non‑medical prescriber or willingness to undertake training.
* Current or aspiring member of the NHS Pension Scheme.
Person Specification
* Active and up‑to‑date registration with the UK Nursing & Midwifery Council (NMC) with a valid membership number.
* Evidence of completed training in BLS, Anaphylaxis, AED, IPC and Safeguarding Level 3.
* Experience in delivering primary care nursing within a multidisciplinary team.
* Strong communication, personal, organisational and clinical assessment skills.
Security and Background Checks
The successful candidate must meet security requirements (security check) and pass a Disclosure and Barring Service (DBS) check under the Rehabilitation of Offenders Act.
Hepatitis B serology testing is required before offer of employment.
Eligibility for Sponsorship
As the role does not qualify for a skilled worker visa, candidates requiring UK visa sponsorship are not eligible to apply.
