An exciting opportunity to join the SMASH Care Community has arisen. Come and join our amazing, friendly, and supportive team! Your role will be to assess, manage, and provide treatment to a caseload of patients with complex and/or multiple therapy needs. You will work closely with the MDT and provide supervision to therapy assistants, junior staff, apprentices, and students. There will be an expectation to work across multiple sites in the organisation, including other care communities and within the discharge‑to‑assess beds. You will also be expected to join the 7‑day, urgent community response rota on a rolling basis covering the five care communities. This is a permanent post. We are looking for an enthusiastic, motivated, and flexible physiotherapist to join our friendly, supportive, and dynamic team within the SMASH Care Community.
Responsibilities
* Undertake comprehensive physiotherapy assessment, provide a therapeutic diagnosis, and implement agreed treatment plans, including the treatment of complex service users with multiple pathologies, respecting privacy, dignity, and individuality.
* Provide individual or group therapy as appropriate.
* Implement monitoring and evaluation of treatment to measure progress, outcomes, and effectiveness.
* Develop programmes of care to support discharge from service.
* Be responsible for equipment used in clinical duties, ensuring safe use by others through teaching, training, and supervision.
* Record, maintain and store accurate, comprehensive documentation in line with legal and local requirements.
* Communicate outcomes of assessments/interventions via letters, reports or discharge summaries.
* Arrange appointments, work closely with MDT, attend case reviews, conferences, and internal/external meetings.
* Contribute to safeguarding of service users and families as appropriate.
* Assess capacity, gain valid informed consent and work within a legal framework with service users lacking capacity.
* Maintain professional registration with HCPC and comply with HCPC and CSP guidelines.
* Maintain knowledge of national and local standards and monitor quality of practice.
* Work as part of a team to ensure effective communication and delivery of care.
* Provide and receive sensitive information with care, integrity and manage contentious information appropriately.
* Work empathetically in partnership with service users, carers and families, adapting communication strategies (e.g. sign language) to involve the service user fully.
* Communicate effectively with service users and carers to progress rehabilitation programmes, persuading/influencing where necessary.
* Use appropriate communication skills to overcome barriers to acceptance of sensitive information.
Leadership and Management
* Communicate effectively with professionals within the organisation and external agencies.
* Provide information to Band 7 physiotherapists / managers for service planning and development.
* Assist managers in achieving departmental objectives, supporting an open leadership style.
* Actively participate in the professional development programme for the therapy service.
* Provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals.
* Take responsibility for daily workload allocation within the specialist area.
* Participate in operational planning, policy implementation and service development, leading delegated projects.
* Undertake duties considered appropriate by senior staff and deputise in their absence.
* Be aware of the financial impact of clinical decisions and support stock management policy.
Education and Professional Development
* Organise and lead in‐service training programmes for Band 5 therapists and assistants.
* Critically evaluate current research and apply it to practice.
* Demonstrate understanding of national guidelines and legislation relating to health and social care.
* Train, supervise and support less experienced staff, assistants and students, including formal appraisals.
* Maintain competency through internal and external development opportunities and a CPD portfolio.
* Demonstrate specialist skills and knowledge to maintain fitness to practice as a physiotherapist.
* Comply with HCPC Code of Conduct and internal policies.
* Keep up to date with mandatory training requirements.
* Participate in evaluation of clinical practice and disseminate findings.
Research, Audit and Governance
* Undertake measurement and evaluation of practice using evidence‑based projects, audits and outcome measures.
* Represent the service at local meetings, specific interest groups and national meetings under guidance.
* Recommend and implement change under supervision.
* Contribute to regional or national audit and research activity.
* Disseminate audit and evaluation findings to relevant areas.

Qualifications and Experience
* Degree in physiotherapy.
* HCPC registered.
* CSP registered (desirable).
* Previous experience working in the NHS.
* Experience working in a community setting.
* Completion of Band 5 rotation (desirable).
* Student educator or experience of having students (desirable).
* Experience of audits (desirable).
* Good written and verbal communication skills.
* Good IT skills (desirable).
* Computer literate with proficiency in Microsoft Office and accurate keyboard skills.
* Car driving licence and access to own vehicle.
