Overview

The successful candidate will work as part of a dynamic and flexible multidisciplinary team, delivering high-quality, responsive care to patients across the Virtual Ward. The role also includes in-reach responsibilities within the acute hospital and Accident & Emergency departments to identify patients suitable for step-up or step-down care within the Virtual Ward. There is an expectation of flexibility to work across the Virtual Ward, Urgent Community Response, and acute hospital settings, supporting seamless transitions of care and ensuring patients receive the right care, in the right place, at the right time.


Key Requirements

* Registered Nurse with current NMC registration
* Level 7 Advanced Health Assessment qualification
* V300 Non-Medical Prescribing qualification, with a minimum of 12 months prescribing experience
* Proven clinical expertise in managing patients with complex needs/acute experience/managing the acutely unwell patient
* Excellent communication, clinical decision-making, and leadership skills
* Ability to work autonomously and collaboratively across services

As a Band 7 Nurse Practitioner within the Virtual Ward, you will deliver autonomous, advanced-level clinical practice that is firmly grounded in direct, compassionate care for patients, families, caseloads, and populations. Your role will encompass elements of clinical leadership, education, research, and service development, while maintaining a strong focus on patient-centered care.

Deliver high-quality, autonomous care to patients within the Virtual Ward, using advanced clinical assessment and decision-making skills.

Monitor patients remotely using a variety of digital platforms; therefore, strong IT skills and digital literacy are essential.

Identify and manage patients suitable for step-up or step-down care between acute, urgent, and community settings, including in-reach to Accident & Emergency and acute hospital wards.

Promote public health and wellbeing through the ‘Making Every Contact Count’ approach, supporting individuals, carers, and families to make positive changes to their physical and mental health.

Support and mentor junior staff, contributing to their development and the overall capability of the team.

Developing and delivering teaching sessions to junior members of the team.

Actively participate in service improvement initiatives, research, and audit activities to enhance clinical practice and patient outcomes.


Accountabilities

* Ensuring safe and effective clinical practice through evidence-based care and adherence to professional standards.
* Enhancing the patient experience by delivering compassionate, responsive, and personalised care.
* Contributing effectively to the organisation’s objectives, including integrated care delivery and admission avoidance.
* Efficient and effective use of resources, ensuring value for money and sustainability.
* Ability to request and interpret clinical investigations (i.e., bloods, ECG, etc).
* Flexibility in working across the Virtual Ward, Urgent Community Response, and Accident & Emergency Department, supporting seamless transitions of care under the front-door model.
