An opportunity has arisen for an enthusiastic individual who is seeking a career in our Carers and Community Support Team as a Team Around the Person Co-ordinator. This role is funded by the Regional Integration Fund and is Fixed Term until 31st March .
The role of the Team Around the Person Co-ordinator aims:
1. To develop systems within communities for the co-ordination of preventative and early intervention support for vulnerable adults.
2. To work with community organisations to develop their networks and draw together organisations that can offer support for community members.
On a day to day basis you will:
3. Work alongside partner organisations in identifying need and innovative solutions to meet need within communities.
4. Develop processes, and protocols that will enable the co-ordination of services.
5. Co-ordinate the work of a multi-agency Team Around the Person.
We are looking to recruit an individual, who:
6. Has the ability to think creatively and innovatively and translate ideas into actions effectively.
7. Is able to develop productive professional relationships with key stakeholders.
8. Has experience of developing new systems of practice.
9. Has experience of leading on Community Based projects.
In addition to the above, we require an individual with a degree of fluency in the Welsh language. A successful applicant without the required Welsh language skill will be fully supported to reach the desired standard within two years of appointment.
