Are you passionate about supporting patients in regaining independence or receiving compassionate end-of-life care? Our nurse and therapist led Intermediate Care Unit offers a rewarding setting, focused on rehabilitation and reablement, with six Specialist Palliative Care inpatient beds. We use a team nursing and therapy approach, delivering person-centered care, with support workers working directly alongside qualified nurses or therapists in a collaborative environment.
This Band 3 role is ideal for experienced support staff. If you are less experienced, you can start as a Band 2 Rehabilitation Support Worker or Healthcare Assistant, with a clear development pathway to progress to Band 3 through training and supervised practice.
This is a 24/7 service and requires availability for nights, weekends, and bank holidays.
The post holder will be part of a dedicated team of support workers trained in a range of essential clinical and care skills. Working under the supervision of registered professionals, they will provide direct, hands-on care and contribute to delivering high-quality, person-centred care that supports both rehabilitation and end-of-life needs.
You will play an active role in helping patients regain independence, as well as providing comfort and support in the final stages of life. You will be expected to deliver delegated care and clinical interventions in line with your training and competencies.

Catheter care and bladder scanning
Recording clinical observations, including NEWS2 and blood glucose testing
Supporting physiotherapy and occupational therapy programmes within your scope of practice

The Unit presently comprises of 20 intermediate care beds delivering rehabilitation and 6 Specialist Palliative Care (SPC) beds in a purpose designed setting. For our intermediate care patients our aims are to reduce acute hospital admission and facilitate early discharge by utilising the skills of the multidisciplinary team of nurses and therapists. For the SPC patients we aim to provide holistic symptom management and end of life care.

Follow an agreed care plan offering a range of clinicalactivities to ensure best practice is delivered.
Undertake personal care activities of daily living with patients.
Support and deliver aspects of activities of daily living.To include management of dietary and fluidintake,toenail care, management of continence, assisting with toileting programmes,includinguse ofsheaths, catheters, and continence pads.
Undertake observations of skin assessments, continence,nutrition, and report back to senior colleagues.
Be ableto participate in simple wound care, skin tear pathway, venepuncture, orcollection of specimens.
Participate in Health Education/training for patients, families,and carers.
Maintain legible, accurate and contemporaneous patientrecords in accordance with ECCH Policy.
Report any changes in patients condition to a registeredHealth Care Professional.
