Clinical Lead

This is an exciting role for a band 7 Clinical Lead to join the Central Home Treatment Team. The Central Home Treatment Team offers 24-hour support to service users, and their families/carers, who are suffering from mental health concerns that would otherwise require admission to an acute inpatient environment. There is a strong focus on a recovery model and effective risk management to provide quality care in a community setting.

The Central Home Treatment Team are always striving towards excellence in service delivery and ensuring that the Trust values are achieved. The successful applicant will be dynamic and forward-thinking with the ability to work as part of a team and in partnership with local services.

There will be an expectation that the successful applicant is able to work a flexible shift pattern across 7 days a week, to provide clinical support, upskill and guidance to junior members of the nursing team including the completion of formal clinical supervision reviews in accordance with the Trust policy.


Main duties of the job

* Provide expert clinical knowledge and interventions, advice, support and supervision in the Home Treatment Team (HTT) to colleagues, service users and carers.
* To work in conjunction with the Team Manager to ensure that the budget is being used effectively and identify/review any cost improvements to ensure a high quality service is being delivered.
* To provide leadership for the team as clinical shift leader, role includes assessment officer, facilitating daily MDT HTT meeting, leading and supporting discharge planning & coordination, providing clinical supervision for HTT practitioners, supporting face‑to‑face gate‑keeping, leading on shift coordination.

Person Specification


Education/Qualifications

* Registered Nurse, Social Worker or Occupational Therapist with Post-registration experience and/or evidence of formal courses and training at post graduate diploma level Clinical Supervision Training


Knowledge/Experience

* Significant demonstrable post qualification experience at Band 6 level, or equivalent, working in a mental health / specialist setting or equivalent.
* Experience of developing, delivering and evaluating specialist programmes of care and risk management plans.
* Evidence of successful mentoring and/or supervision of junior members of staff including students.
* Evidence of improving service delivery in line with evidence based practice.
* Demonstrable experience of effective multi-disciplinary team working.


Skills and Abilities

* Highly developed communication and interpersonal skills and the ability to demonstrate diplomacy, tact and negotiation skills.
* Ability to manage highly stressful and emotive situations.
* Current knowledge of the policy and challenges facing NHS providers.
* Up to date and relevant knowledge of evidence based practice, e.g. NICE Guidance.
* Knowledge of regulatory bodies and the requirements associated with this in terms of standards of service delivery, e.g. CQC.
* Ability to develop, implement and evaluate specialist programmes of care and risk management plans.


Personal

* Acts as a role model in the delivery of high quality, effective patient care.
* Ability to liaise with representatives from other organisations and to communicate a positive image of self and of the service/organisation.
* Demonstrate evidence of team working.
