Role Overview

Omni-Care Pty Ltd is a leading provider of high quality home based support services for elderly people or individuals with serious medical conditions. We are known for flexible and responsive support and excellent customer service. We have been providing home care packages since 2017 and are growing our aged care services through Home Care Packages and Support at Home.


The Role

We are looking for an energetic and committed Aged Care Manager to join our leadership team. The role is responsible for managing a team of seven care partners and an administration assistant to ensure excellent outcomes for our Home Care Packages clients. This position reports to the General Manager and is based in our Preston office. Our Home Care Package clients are located throughout the Northern, Western and Eastern suburbs of Melbourne.


Key Responsibilities

* Exceed customer service expectations in the care management, service delivery and administration of home care packages.
* Oversee the management of home care packages within budget and in compliance with the Aged Care Quality Standards.
* Ensure the care management team provides client support that reflects industry best practice, focuses on wellness and reablement, and is responsive to individual needs through ongoing monitoring, supervision and guidance.
* Oversee business development opportunities for the growth of Omni-Care Home Care Packages and Support at Home in the future.
* Monitor and evaluate the effectiveness of the service model and related systems and processes; identify continuous improvement opportunities.


Success in this role will require

* Demonstrated experience in a hands-on leadership or management role in aged care, preferably Home Care Packages.
* Comprehensive understanding of the Aged Care Quality Standards and regulatory/compliance requirements.
* Exposure to and understanding of the Support at Home program (to the extent it has been finalised).
* Excellent written and oral communication skills with the ability to negotiate positive outcomes.
* Relevant tertiary qualifications in aged care, health care or related fields.
* Experience with aged care management systems including the My Aged Care Portal and the Microsoft Office suite.
* A current driver’s license, a clear national police check or NDIS worker screening check, and permanent Australian work rights.
