Overview
Liaison Care, part of Liaison Group, is expanding its team to support National NHS Continuing Healthcare (CHC) reviews and assessments. The role involves conducting remote CHC reviews and assessments, chairing MDT meetings, and working within the NHS CHC framework and the Care Act 2014. This is a remote-friendly opportunity with a comprehensive induction and buddy programme, and an excellent remuneration and benefits package.
Location: Worcester, Worcestershire (Liaison Court, Vincent Road, WR5 1BW). Salary: £38,000 to £44,962 per year. Contract: Permanent. Working pattern: Full-time, Part-time, Home or remote working.
Responsibilities
* Lead Multi-Disciplinary Team (MDT) meetings.
* Complete either the NHS Continuing Health Care (CHC) Decision Support Tool or the Adult Needs Assessment with clear eligibility recommendations.
* Complete CHC reviews.
* Manage follow-up calls and request additional documentation and evidence from other professionals and care providers as required.
* Conduct assessments, reviews, and care and support plans, working to Liaison Care and ICB KPI requirements.
Qualifications
Essential
* NMC Registered Nurse.
* Minimum 12 months post-qualification experience.
* Previous experience within an NHS Continuing Health Team or in the community completing CHC assessments and reviews.
* Experience chairing MDT meetings.
* Ability to write clear and comprehensive reviews and assessments to a high standard.
* Ability to develop person-centred care and support plans.
* Computer literate, including Microsoft Outlook and Excel.
* Experience in similar work: reviewing, evaluating, and planning adult health and social care.
* Good working knowledge of CHC Framework 2022.
* Knowledge of Care Act 2014 and Mental Capacity Act 2005.
