Qualified Social Worker, Hospital Discharge to Assess Team, Manchester City Council Pay rate to £30 per hour Contract role Vitalis are recruiting for experienced Social Worker to work in a Hospital Discharge to Assess Central Community Team in Manchester City Council. MCC require an agency social worker to support the HDTA Central Community Team for a period of three months to cover a vacancy. Social worker with significant experience in adult social care required, this is a 7 days service, which includes weekend working as per rota.
